  • Expanded access to care by providing DSME services beyond the pharmacy, including assisted living facilities, independent living communities, libraries, clubs, and other community settings

  • Strengthened community health outcomes by making diabetes education inclusive, sustainable, and patient-centered

  • Dr. Chau Phan and Dr. Linh Bui have been dedicated pharmacists since 1991, bringing more than three decades of clinical excellence, compassionate care, and community leadership to the Vietnamese American community in Orange County. Their careers span high‑volume community pharmacies, specialty services, and the development of patient‑centered care models that emphasize accessibility, cultural understanding, and professional integrity. As Pharmacists‑in‑Charge, they have guided operations, mentored staff, and helped shape ProCare Pharmacy, LCScripts Pharmacy, and MedHealth Pharmacy into trusted healthcare destinations.
